Group Medical Insurance (New Summary Effective 10/01/2012 In Area / Out of Area)

§        The PGCPS medical plan is called Choice+ which is administered by CareFirst BlueCross BlueShield

§        Choice+ is a triple-option plan which includes three levels of coverage:

§        Option 1 (HMO),

§        Option 2 (PPO),

§        Option 3 (Traditional fee for service Indemnity coverage)

§        The three levels of coverage are available at all times

§        Voluntary plan whereby employees must enroll within 60 days from date of hire or during Open Enrollment using Oracle Self Service

§        PGCPS pays 75% of monthly premium for medical insurance, the PGCPS share increases to 80% after 8 years of service.  The employee pays the remainder through payroll deduction

§        Premiums are deducted on a pre-tax basis
